Bug Fixes in Graphics

Tobias Koenig committed changes in /trunk/playground/graphics/okular/generators/chm:

* Fixed the loading of chm documents (avoids crashes)
* Improved performance by creating text page/object rects only on first generatePixmap call
* Code cleanup
